Admission to Medicine Floor:
- Vital Signs every 4 hours
- Note: Allergic to penicillin, watch for orthostatic hypotension
- Intravenous (IV) Fluids: 2 L Normal Saline (NS) wide open; then 1/2 NS at 100 cc/hr if no longer experiencing orthostatic hypotension
- Hyperglycemia/Type 1 Diabetes Mellitus (DM) Control:
- Fingerstick (FS) glucose check every 2 hours until below 200 mg/dL; then every 4 hours
- Insulin Lispro administered pre-meals with a sliding scale increase for every blood sugar level above 150, considering patient’s food intake
- Continue routine long-acting insulin
- Monitor urine for ketones
- Pyelonephritis Treatment:
- Initiated Levofloxacin (Levaquin) 750 mg IV every 24 hours for five days
- On day five, switched to oral levofloxacin 750 mg PO daily for nine days, completing a total 14-day antibiotic treatment duration.
